Filling this out will give you and us a record of when this was requested was made to ensure it will be taken care of completely. An email (directed to the one filled out in the form) will be sent you time stamping the request and verifying that we received it. Keep a copy of this in your email for your records. Within 24 hours you will receive another email from answers@medical-ins.com describing where in the process it is and when it will be completed. After your request has been completed another email will be sent verifying the completion. Use these emails for your records!
